Transition Nurse launch
Carlisle Mencap’s Grace Little Centre hosted the launch of the new WellChild Specialist Transition Nurse for the Carlisle and north Cumbria area.
BBC and ITV were among the press covering the launch of the post which is jointly funded by Cumbria Partnership Foundation NHS Trust and Wellchild, the national charity for sick children.

There are 42 specialist nurses nationwide and each nurse has a launch in their locality to promote the valuable support & expertise that they provide to children with complex health needs and their families.
The Wellchild Specialist Transition Nurse post is unique, in that the nurse provides specialist advice and support to the Community Children’s Nurses who are familiar to the young people and their families, supporting them through the complex process of transition to adult services. The key is early intervention to empower families to plan with the young person their wishes and aspirations for their future. Providing information to ensure they have choices and options.
Nurses provide essential care and practical support to children and young people with exceptional health needs and their families. They play a crucial role in enabling children to leave hospital and be cared for at home, reducing the practical, emotional and financial impact for families.
